Job Summary

We are seeking a dedicated and experienced Registered Nurse Case Manager to join our healthcare team. This vital role involves coordinating patient care across various settings, ensuring seamless transitions from acute care to post-acute and outpatient services. Duties

• Conduct thorough patient assessments, including vital signs, physiology evaluation, and medical history reviews to determine individualized care plans.
• Coordinate inpatient and outpatient care services across hospital, clinic, and post-acute settings to facilitate smooth patient transitions.
• Develop and implement comprehensive discharge plans that incorporate post-acute care options, home health services, and community resources.

• Manage utilization review processes by evaluating diagnostic evaluations, medical necessity, and resource allocation in compliance with HIPAA regulations.

   

Requirements

• Min 2 years of RN Case Manager experience required
• PRI (Patient Review Instrument) Certification, SCREEN Training Certification
• BLS
