Description
Job Title: Licensed Practical Nurse (LPN/LVN) – Long Term Care / Skilled Nursing
Location: Claremont, NH
Job Type: 13 Weeks Contract
Job Summary:
The Licensed Practical Nurse (LPN/LVN) provides direct patient care in a Long Term Care/Skilled Nursing setting under the supervision of an RN or Nursing Leadership. This role includes medication administration, monitoring resident conditions, supporting care plans, and supervising nursing assistants to ensure quality patient care and compliance with facility policies.
Key Responsibilities:
- Monitor residents’ physical and mental status and report changes to RN/Supervisor
- Administer medications, treatments, oxygen therapy, and tube feedings per physician orders
- Perform wound care and assist with wound vac management
- Accurately document care in EMR, MARs/TARs systems
- Supervise and support LNAs in delivering patient care
- Assist with admissions, discharges, and transfers
- Participate in physician rounds and care coordination
- Maintain infection control standards and a safe environment
- Communicate effectively with residents, families, and interdisciplinary teams
- Complete shift reports and ensure continuity of care
- Follow protocols for medication storage, administration, and controlled substances
- Assist with staff orientation and training as needed
Requirements:
- Minimum 3+ years of recent LPN/LVN experience (within the last 6 months) – Required
- Experience in Long Term Care / Skilled Nursing – Required
- Must be willing to float to units where experienced
- IV Certified (or willing to obtain during onboarding) – Required
- Ability to work in a rural / small facility setting
- Previous travel experience – Highly Preferred
Certifications:
- BLS (AHA) – Required
Licensure:
- Active New Hampshire LPN License or Compact License – Required
- NH License must be active
