Job Title: Physical Therapist Assistant (PTA) – Home Health
Location: San Jose / South Bay / Santa Clara County, California
Job Type: Travel / Contract
Job Summary:
The Physical Therapist Assistant (PTA) provides skilled patient care in a Home Health setting under the supervision of a licensed Physical Therapist. This role focuses on delivering therapeutic interventions, improving patient mobility, and supporting recovery in a home-based environment.
Key Responsibilities:
- Assist Physical Therapists in implementing individualized care plans
- Provide therapeutic exercises, gait training, balance training, and mobility interventions
- Deliver care to patients in home health settings across assigned coverage areas
- Monitor patient progress and report updates to supervising Physical Therapist
- Educate patients and caregivers on home exercise programs and safety
- Maintain accurate and timely documentation in EMR systems
- Ensure compliance with infection control and safety protocols
Requirements:
- Minimum 2 years of experience as a Physical Therapist Assistant (PTA)
- Home Health experience – REQUIRED
- Ability to work independently in a home-based setting
- Strong communication and organizational skills
Certifications:
- BLS (Basic Life Support) – Required
Licensure:
- Active California PTA License – Required
Schedule:
- 5x8 shifts (8:00 AM – 5:00 PM)
