Job Title: Registered Nurse (RN) – Nurse Assessor | Hybrid | NYC Five Boroughs

Location: New York City, NY (Five Boroughs + 50 Mile Radius)

Job Summary:

We are seeking an experienced Registered Nurse (RN) Nurse Assessor for a hybrid role supporting consumers seeking community-based long-term care services throughout the New York City metro area. This position includes a mix of telehealth assessments and in-home visits across the Five Boroughs and surrounding areas.

The ideal candidate will have strong home health experience, assessment skills, and the ability to work independently in the field.

Responsibilities:

• Conduct in-home and telehealth nursing assessments for long-term care consumers
• Evaluate health status, functional abilities, care needs, and support systems
• Review medical records, referrals, and supporting documentation
• Complete and submit assessments in the UAS-NY system
• Document call outcomes, appointments, and care findings accurately
• Collaborate with clinical quality teams regarding non-qualified assessments
• Maintain knowledge of community resources and support services
• Travel throughout assigned NYC regions for scheduled home visits

Requirements:

• Active Registered Nurse (RN) License – New York State
• Minimum 2 years recent Home Health RN experience
• Strong assessment and documentation skills
• Experience with geriatric and chronic disease patients preferred
• Comfortable with telehealth platforms, Outlook, Word, and web-based systems
• Valid New York State Driver’s License or ID
• Reliable transportation preferred

Preferred Qualifications:

• UAS-NY assessment experience
• MLTC / Managed Care experience
• Bilingual skills highly preferred (Spanish, Russian, Chinese, Korean, Haitian Creole)

Schedule / Work Setting:

• Hybrid role: Remote telehealth + field home visits
• Day shift
• Local travel required throughout NYC boroughs and surrounding areas
