Description

We are seeking an experienced Travel Home Health Registered Nurse (RN) to support patient care in the Atlanta, GA area. This role focuses on delivering high-quality, in-home nursing care, performing comprehensive assessments, and managing patient treatment plans in a fast-paced home health setting.

Job Title: Travel Registered Nurse (RN) – Home Health

Location: Atlanta

Coverage Areas: Gwinnett, DeKalb, Fulton Counties

Assignment Details

  • Duration: Contract (Travel)
  • Weekly Guarantee: 32 hours
  • Schedule: Monday – Friday, 8:00 AM – 4:30 PM
  • On-Call/Weekend: Required once per month
  • Mileage Reimbursement: Yes (between patient visits)

Key Responsibilities

  • Perform skilled nursing visits in patients’ homes across assigned territories
  • Complete and document OASIS assessments accurately and efficiently
  • Manage a weekly productivity expectation of 30 points minimum
  • Develop, implement, and evaluate individualized patient care plans
  • Monitor patient conditions and communicate changes to physicians and care teams
  • Administer medications and treatments as prescribed
  • Utilize Homecare Homebase (HCHB) for documentation and care coordination
  • Educate patients and families on disease management and care plans
  • Ensure compliance with agency policies and healthcare regulations

Required Qualifications

  • Active Registered Nurse (RN) license – State of Georgia
  • Proven Home Health experience
  • Strong experience with:
    • OASIS documentation
    • Homecare Homebase (HCHB)
  • Valid driver’s license and reliable transportation
