Job Overview
The RN – Home Health provides direct patient care in the home setting. This role includes patient assessment, care planning, and coordination with other healthcare services to ensure quality and continuity of care.
Key Responsibilities
- Provide in-home nursing care to patients
- Perform patient assessments and develop care plans
- Coordinate care with PT, OT, social work, and other services
- Monitor patient progress and update care plans
- Educate patients and families on care and treatment
- Maintain accurate documentation
- Manage daily patient visits and travel between locations
Requirements
- Minimum 1 year of Home Health RN experience (REQUIRED)
- Strong communication and patient care skills
- Ability to work independently in the field
- Comfortable with traveling between patient homes
Certifications Required
- BLS (AHA)
- Active Michigan RN License
Education
- Graduate of an accredited nursing program
