Job Summary:
 

The Speech Language Pathologist (SLP) provides skilled speech therapy services in a Home Health setting. This role focuses on evaluating, diagnosing, and treating communication and swallowing disorders while collaborating with interdisciplinary teams to develop and implement individualized care plans.
 

Key Responsibilities:

• Provide direct speech therapy services to patients in a home health setting
• Evaluate and treat speech, language, cognitive, and swallowing disorders
• Develop and implement individualized treatment plans
• Educate patients and caregivers on therapy techniques and home programs
• Collaborate with physicians and interdisciplinary teams
• Maintain accurate and timely documentation
• Utilize rehabilitation equipment such as communication aids and assistive devices
• Work independently while managing patient caseload efficiently

Requirements:

• Minimum 2 years of Home Health SLP experience – Required
• Ability to work independently with minimal supervision
• Valid Driver’s License – Required
• Must have own vehicle (mandatory for this role)
• Acute care experience – Preferred

Education:

• Master’s or Doctorate Degree in Speech Language Pathology – Required

Certifications & Licensure:

• CCC (Certificate of Clinical Competence) – Required (Clinical Fellows accepted)
• BLS (AHA) – Required
• Active Massachusetts SLP License – Required
