Description
Job Summary:
We are seeking a Pharmacist to provide medication therapy management and clinical support in a home infusion setting. This role focuses on patient safety, clinical consultation, and collaboration with healthcare teams to ensure effective treatment outcomes.
Responsibilities:
- Review and verify prescriptions for accuracy and safety
- Provide drug information and clinical guidance to healthcare providers
- Monitor drug interactions, contraindications, and adverse events
- Support medication therapy management and patient education
- Assist with pharmacy workflow, inventory, and storage processes
- Collaborate with physicians, nurses, and insurance providers
- Participate in audits, compliance, and quality improvement initiatives
- Perform clinical services such as immunizations and disease management
- Support and guide pharmacy support staff
- Maintain high standards of customer service and patient care
Requirements (Must Have):
- Active South Carolina Pharmacist License
- PharmD or Bachelor’s Degree in Pharmacy
- Minimum 1 year of pharmacist experience
- Strong knowledge of medical terminology
- Excellent communication, problem-solving, and teamwork skills
Preferred Qualifications:
- Experience in home infusion or hospital pharmacy
- Experience with Epic system
- Experience working with insurance and prior authorizations
