Description
Pride Health is hiring a Home Health Physical Therapist to support our clients in the medical facility. This is a temporary assignment opportunity and a great way to start working with a top-tier healthcare organization!
Job Title - Home Health Physical Therapist
Job Type: Contract
Location- Aberdeen, WA
Shift: Monday–Friday 8:00 am to 4:30 pm/ 40 hrs/ week
Mileage reimbursement provided (from first patient to last patient visit)
Job Summary:
We are seeking an experienced Travel Physical Therapist (PT) to join a home health team in Aberdeen, WA. This role involves delivering high-quality, patient-centered care in a home setting, performing evaluations, developing care plans, and supporting patient recovery and mobility.
Key Responsibilities:
- Perform Start of Care (SOC), Evaluations, Resumption of Care (ROC), and Recertifications
- Provide ongoing therapy visits and reassess patient progress
- Develop and implement individualized treatment plans to improve mobility and function
- Educate patients and caregivers on therapeutic exercises and safety practices
- Maintain accurate and timely clinical documentation
- Collaborate with interdisciplinary healthcare teams to ensure optimal patient outcomes
- Ensure compliance with home health regulations and agency policies
Required Qualifications:
- Active Physical Therapist (PT) license (state-specific)
- Doctor of Physical Therapy (DPT) required
- Current CPR certification
- Prior Home Health experience
- Strong clinical assessment and patient care skills
- Ability to manage an independent, field-based workload
Certification:
- Valid PT License
Preferred Qualifications
- Experience with Homecare Homebase (HCHB) EMR
- Strong time management and organizational skills
- Excellent communication and patient education abilities
