Specialty: Hospice RN (Home Hospice)
Shift: 8-hour Day Shift (40 hrs/week)
Schedule: Monday–Friday, 08:00–16:30
Travel Type: Home Health / Hospice Field RN

Facility & Setting

• Home Hospice RN role (patient care delivered in patient homes)
• Quarterly in-person team meetings at Trinity Hospital
• Initial onboarding/supply pickup at Oak Brook regional office
• Coverage area: Lynwood, Calumet, Calumet City, Calumet Park, Riverdale, Dolton
• No weekends, no on-call, no holiday requirements
• Free parking provided

Patient Population

• Adult hospice patients only
• Diagnoses include:
  • End-stage COPD
  • Advanced cancers
  • Failure to thrive
  • Terminal illnesses and end-stage disease processes
• Average caseload: 12–15 patients per RN

Experience Requirements

• Minimum 3 years Home Hospice or Home Health experience required
• First-time travelers accepted

Required Credentials

• BLS certification
• Valid driver’s license
• Auto insurance
• Reliable personal vehicle

Clinical Skills Required

• Wound care (including wound vac management, measurement, and weekly photo documentation)
• Drain care (Pleurex, nephrostomy, Foley)
• Pain and symptom management
• End-of-life care planning
• Patient and family education
• Care coordination and triage

Soft Skills

• Strong organization
• Time management
• Independent field work capability

Support & Equipment

• Company-provided phone and laptop
• Trunk stock supplied
• Clinical and management support available by phone
• EMR: EPIC
• Orientation: 1–2 weeks based on experience
