• Float Requirements: YES. Will be floating between Kindred Chicago Northlake and Kindred Chicago North

• Number of Beds: Northlake 50 beds, North 85
• Patient Ratios: Average of 12 treatments and 2 evals per day.
• Equipment: Hoyer, Sara Plus, Hi/Low mat table, stairs, NuStep
• EHR: Protouch, Net Health for Lifepoint billing

• Supervisor: Karen Supovec at Northlake, Rachel Dargatz at North

• Patient Population: LTACH

Hiring Profile:

• Must Have Skills/Experience: at least 1 year of OT experience with adults
• Preferred Skillset/Experience: LTACH or ICU, med/surg
• Common Diagnosis/Treatment: respiratory failure, trach, vent, high flow O2, wounds, TBI, Stroke, burns, etc
• Personality Fit: efficiency, able to collaborate with other disciplines

• Required Education: OT licensed
