Required Qualifications

• Valid RN license in New York
• Minimum 3 years of Medical/Surgical nursing experience, including experience within the last year
• BLS certification issued by the American Heart Association
• ACLS certification issued by the American Heart Association
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Experience in stroke care

Health and Immunization Requirements

• COVID-19 Vaccine: Optional; proof of vaccination or agency declination accepted
• MMR: Positive IgG titer or 2 vaccine series required; if negative/equivocal, vaccination must be completed within 30 days of start
• Varicella: Positive IgG titer or 2 vaccine series required; if negative/equivocal, vaccination must be completed within 30 days of start
• Hepatitis B: Positive antibody titer, 3 vaccine series, or specific declination form required
• Influenza Vaccine: Required or specific declination form; if declined, mask must be worn at all times on assignment
• Tdap: Preferred but not required; if no record, specific declination form required
• TB Testing:

  • One of the following within last 12 months:

    • Two PPD skin tests (one within 12 months, one within 3 months)
    • One Quantiferon/Spot test within 12 months
  • For past positive TB tests, chest x-ray within 12 months may be required
• Fit Test: Must be completed within last 12 months prior to start; specific approved mask models required; on-site fit testing available if needed
• Physical/Health Statement:

  • Within last 12 months
  • Must include physician credentials, clinic location, phone number
  • Must document height, weight, vitals, Snellen eye chart
  • Statement confirming employee passed physical with no restrictions

Additional Requirements

• Color headshot photo (professional, no filters, solid background)
• Completion of agency-specific forms and acknowledgments (e.g., code of conduct, sexual harassment acknowledgment, parking registration)
• Completion of agency-specific self-study packet and quiz
• Arnot Epic Questionnaire (submission required)
• TB Questionnaire Form (submission required)

• Prior travel experience is not explicitly required for this position.
