Description

Pride Health is hiring a Physical Therapist to support our client’s medical patients based in Fort Myers, FL 33912 (Coverage within Cape Coral and North Fort Myers). This is a 3+ month contract job opportunity and a great way to start working with a top-tier healthcare organization!
 
Job Details:
Job Name: Physical Therapist

Location: Fort Myers, FL 33912, with Travel within Cape Coral and North Fort Myers.

Job Type: LocalContract         

Duration: 3+ months Contract (Possibilities of extension)

Pay rate: $50 - 56/hr. On W2 (Negotiable - Pay Rate is based on experience)

Schedule: Mon – Fri (5 days/ wk.) from 8 AM to 4:30 PM (30-hour weekly guarantee)

Mileage: Reimbursement provided (from second patient to last patient)

 

Job Responsibilities:

  1. Perform Start of Care (SOC) visits, completing initial assessments and establishing individualized Plans of Care (POC) in accordance with physician orders.
  2. Conduct thorough physical therapy evaluations assessing strength, mobility, balance, pain, and functional limitations.
  3. Provide skilled therapy interventions during routine and follow-up (revisit) appointments.
  4. Perform re-evaluations to assess patient progress, update treatment plans, and adjust goals as needed.
  5. Develop and implement evidence-based treatment plans tailored to each patient’s needs and goals.
  6. Educate patients and caregivers on home exercise programs, safety precautions, and disease management.
  7. Complete timely and accurate clinical documentation in compliance with regulatory and organizational standards.
  8. Coordinate care with physicians, interdisciplinary team members, and caregivers to ensure optimal patient outcomes.
  9. Conduct discharge visits, including final assessments, patient education, and transition planning.

 

Qualifications:

  • Graduate of an accredited Physical Therapy program (bachelor’s or higher).
  • Active, unrestricted Physical Therapist (PT) license in New Jersey.
  • Minimum 1 year of clinical experience (home health experience preferred).
  • Valid driver’s license and reliable transportation.
