Description
Pride Health is hiring a Physical Therapist client in the medical facility. This is a temporary assignment opportunity and a great way to start working with a top-tier healthcare organization!
Job Type: Contract
Location- Fort Wayne, IN
Shift: Mon – Fri 8 am – 5 pm
Start Date: 05/04/2026
Coverage Areas: Adams, Allen, DeKalb, Huntington, Kosciusko, LaGrange, Noble Steuben, Wells and Whitley Counties
Key Responsibilities:
- Perform Start of Care (SOC), Evaluations, Resumption of Care (ROC), and Recertifications
- Provide ongoing therapy visits and reassess patient progress
- Develop and implement individualized treatment plans to improve mobility and function
- Educate patients and caregivers on therapeutic exercises and safety practices
- Maintain accurate and timely clinical documentation
- Collaborate with interdisciplinary healthcare teams to ensure optimal patient outcomes
- Ensure compliance with home health regulations and agency policies
Required Qualifications:
- Active Physical Therapist (PT) license in Indiana
- Doctor of Physical Therapy (DPT) required
- Current CPR certification
- Prior Home Health experience
- Strong clinical assessment and patient care skills
- Ability to manage an independent, field-based workload
Certification:
- Valid PT License
Preferred Qualifications
- Experience with Homecare Homebase (HCHB) EMR
- Strong time management and organizational skills
- Excellent communication and patient education abilities
