Pride Health is hiring a Physical Therapist client in the medical facility. This is a temporary assignment opportunity and a great way to start working with a top-tier healthcare organization! 

Job Type: Contract

Location- Fort Wayne, IN

Shift:   Mon – Fri 8 am – 5 pm

Start Date: 05/04/2026

Coverage Areas: Adams, Allen, DeKalb, Huntington, Kosciusko, LaGrange, Noble Steuben, Wells and Whitley Counties

Key Responsibilities:

• Perform Start of Care (SOC), Evaluations, Resumption of Care (ROC), and Recertifications
• Provide ongoing therapy visits and reassess patient progress
• Develop and implement individualized treatment plans to improve mobility and function
• Educate patients and caregivers on therapeutic exercises and safety practices
• Maintain accurate and timely clinical documentation
• Collaborate with interdisciplinary healthcare teams to ensure optimal patient outcomes
• Ensure compliance with home health regulations and agency policies

Required Qualifications:

• Active Physical Therapist (PT) license in Indiana
• Doctor of Physical Therapy (DPT) required
• Current CPR certification
• Prior Home Health experience
• Strong clinical assessment and patient care skills
• Ability to manage an independent, field-based workload

Certification:

• Valid PT License

Preferred Qualifications

• Experience with Homecare Homebase (HCHB) EMR
• Strong time management and organizational skills
• Excellent communication and patient education abilities
