🔹 Patient Care Responsibilities

• Provide direct bedside care to residents (mostly geriatric population)
• Monitor and record vital signs (BP, pulse, temperature, respiration)
• Assist with activities of daily living (ADLs):
  • Bathing
  • Dressing
  • Feeding
  • Toileting
• Support mobility (transfers, ambulation, fall prevention)

🔹 Medication Administration

• Administer oral, topical, and some IV medications (depending on state regulations)
• Perform medication passes accurately and on schedule
• Monitor for side effects and adverse reactions
• Maintain proper documentation (MAR – Medication Administration Record)

🔹 Wound Care & Treatments

• Perform basic wound care (dressing changes, skin assessments)
• Provide treatments such as:
  • Catheter care
  • Tracheostomy care (in some facilities)
  • Feeding tube care
• Monitor for pressure ulcers and skin breakdown

🔹 Monitoring & Assessment

• Observe residents for changes in condition and report to RN/physician
• Perform focused assessments (not full head-to-toe like RN in most settings)
• Track intake/output and report abnormalities

🔹 Documentation

• Chart patient care in EMR systems (e.g., PointClickCare, MatrixCare)
• Maintain accurate, timely, and legal documentation
• Record incidents, changes in condition, and care provided

🔹 Coordination & Communication

• Communicate with:
  • RNs
  • Physicians
  • CNAs
  • Families
• Participate in care planning and interdisciplinary team meetings
• Delegate tasks to CNAs and supervise their work

🔹 Admission, Discharge & Transfers

• Assist with new admissions (initial assessments, documentation)
• Prepare residents for hospital transfers or discharge
• Provide basic patient/family education

🔹 Compliance & Safety

• Follow infection control protocols
• Ensure compliance with standards set by Centers for Medicare & Medicaid Services
• Adhere to facility policies and safety guidelines
• Prevent falls, infections, and medication errors

🔹 Typical Work Environment in LTC/SNF

• High patient ratio (often 15–30+ residents per LPN)
• Focus on chronic care, rehab, and elderly patients
• Fast-paced with heavy med pass and documentation workload
