Description
Position: Physical Therapist - Home Health
Location: Medford, OR (travel throughout Jackson County)
Schedule: Mon–Fri, 8:00a–4:30p
Contract Length: 13 weeks
Start Date: May 18, 2026
Additional Details:
- Hours: 40/week with weekly guarantee
- Productivity Expectation: 30 points
- Mileage: Reimbursed between visits (first to last patient)
- On‑Call/Weekends: None
- Dress Code: Business casual or scrubs (any color)
Responsibilities:
- SOCs
- PT evaluations
- Recerts
- ROCs
- Supervisory visits
Requirements:
- Doctor of Physical Therapy (DPT)
Preferred:
- Home Health experience
- HomeCare Homebase EMR experience
