Job Summary:
We are seeking a compassionate and skilled Speech-Language Pathologist (SLP) to evaluate, diagnose, and treat speech, language, cognitive, and swallowing disorders in a long-term care or rehabilitation setting. The ideal candidate will provide evidence-based therapy, maintain accurate documentation, and collaborate with interdisciplinary care teams to support patient progress and recovery.

Requirements

• Minimum 1 year of Speech-Language Pathology experience – Required
• Long-Term Care (LTC) or Rehab experience – Required
• Active Massachusetts State SLP License – Required
• CCC-SLP Certification – Required
• New graduates accepted with Level II clinical experience (not observation-only Level I)

Key Responsibilities

• Perform speech, language, cognitive, and swallowing evaluations using standardized assessments.
• Develop and implement individualized treatment plans based on evidence-based practice.
• Monitor patient progress and modify treatment plans as needed.
• Provide counseling and education to patients, families, and caregivers.
• Maintain accurate clinical documentation and treatment records.
• Record therapy minutes according to regulatory requirements.
• Communicate patient progress and barriers with healthcare team members.
• Prepare discharge summaries and referral recommendations.
• Participate in care conferences, rehab meetings, and required trainings.
• Assist with obtaining necessary DME or therapy-related supplies.
• Supervise and support CFY clinicians or student Speech-Language Pathologists when applicable.

License Requirement

• Active Speech-Language Pathology License in Massachusetts

Preferred Setting

• Skilled Nursing Facility (SNF), Rehab Center, or Long-Term Care Facility
