Position Overview

• Employment Note: Local candidates accepted at the same rate
• Patient Population: Adults with general medical and high-risk diagnoses

Minimum Qualifications

• Experience: Minimum 1 year required
• Certifications: BCLS required
• COVID-19 Vaccine: Required – Medical/Religious Exemptions Only
• Licensure/Other:
  • Valid driver’s license required
  • Must have a personal vehicle for home visits

Required Clinical Skills & Experience

• Skilled home health care experience (private duty nursing alone will not be accepted)
• OASIS training and experience required
• Proficiency in:
  • IV therapy
  • Wound care
  • Post-operative care
  • General medical diagnoses

High-Risk Diagnosis Experience Required:

• Heart Failure (HF)
• COPD
• Myocardial Infarction (MI)
• Pneumonia
• Stroke

Clinical Responsibilities

• Cardiopulmonary assessments
• Basic wound care, including:
  • ABI compression dressing
  • Wound VAC dressing
• Pleurx / Asept drain care
• Foley catheter care and changes
• Staple and suture removal
• Glucometer use
• Pro-time machine use
• SQ/IM injections
• Lab draws (venipuncture)
• IV therapy
• PICC line or Midline care and dressing changes
• SQ port access and de-access

Work Setting & Productivity

• Setting: Skilled Home Care (Medicare, Medicare Advantage, and Commercial Insurance)
• Visits: 6–8 home visits per day
• Scheduling: Visits and hours are self-scheduled based on HomeCare office needs

Schedule & Coverage Requirements

• Shift Options:
  • Monday – Friday: 8:00 AM – 5:00 PM
  • Monday – Friday: 9:00 AM – 6:00 PM
  • Thursday – Monday or Friday – Tuesday schedules
• Weekends: Alternating weekend shifts required

  • BayCare Forms
  • BayCare Scheduling
  • Ace Timesheet documentation system

Additional Requirements

• Must be COVID vaccinated or have an approved exemption on file
• Must have flu vaccine or declination; mask wearing required at all times if declination is on file
• Scrubs: Any solid color
