Description
LPN – LTC (Licensed Practical Nurse – Long-Term Care) Job Description
A Licensed Practical Nurse (LPN) in a Long-Term Care (LTC) facility provides basic nursing care to residents, usually elderly or chronically ill patients, under the supervision of an RN (Registered Nurse) or physician.
Key Responsibilities
- Administer medications and treatments as prescribed.
- Monitor and record vital signs (blood pressure, pulse, temperature, respiration).
- Assist residents with daily living activities such as bathing, dressing, and mobility.
- Observe patients’ conditions and report changes to the RN or physician.
- Maintain accurate patient records and documentation.
- Provide wound care and basic medical procedures.
- Assist with care planning for residents.
- Supervise and support CNAs (Certified Nursing Assistants).
- Ensure patient comfort, safety, and infection control standards are maintained.
Requirements
- Valid LPN/LVN license in the state of practice.
- BLS/CPR certification required.
- 1+ year of LTC or clinical experience preferred.
- Strong communication and patient care skills.
