Job Type: Contract (13 Weeks)

Job Overview:
We are seeking a dedicated LVN/LPN with at least 2 years of recent experience in a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) setting. This is a high-priority 13-week contract role requiring a compassionate professional who can deliver quality patient care, administer medications, monitor patient conditions, and collaborate effectively with the healthcare team in a fast-paced environment.

Requirements:

• LPN / LVN IA state license
• 2 years of recent experience in an LTC /SNF setting.
• BLS through AHA
• COVID card

Duties:

• Provide direct patient care, including monitoring vital signs and assessing patient conditions
• Administer medications and treatments as prescribed by physicians
• Assist with daily living activities and ensure patient comfort and safety
• Document patient care, progress, and any changes in condition accurately
• Collaborate with nurses, physicians, and healthcare staff to deliver quality care

Skills:

• Strong clinical skills in patient care within SNF/LTC settings
• Proficiency in medication administration and treatment procedures
• Excellent patient assessment and monitoring abilities
• Knowledge of EMR/EHR documentation systems
• Effective communication and teamwork skills
