Chronic Dialysis RN – Glens Falls, NY

 

Contract Duration: 13 Weeks (Starts ASAP)

Shift: Days | 12 to 13.5 Hour Shifts Guaranteed Hours: 36 Hours Per Week

 

Job Summary:

 

Seeking an experienced Dialysis RN for their Glens Falls chronic outpatient clinic. This is a high-volume facility with 32 machines serving approximately 134 patients. You will be responsible for the total nursing care of patients with End-Stage Renal Disease (ESRD), including machine setup, vascular access assessment, and real-time monitoring of hemodialysis treatments. DCI emphasizes clinical excellence and requires absolute scheduling flexibility—this clinic does not accommodate blocked schedules or specific RTO requests.

 

Key Responsibilities

  • Hemodialysis Treatment: Initiate, monitor, and terminate dialysis treatments according to facility protocols.
  • Vascular Access Management: Assess and manage various access points, including Arteriovenous (AV) Fistulas, Grafts, and Central Venous Catheters (CVC).
  • Patient Monitoring: Closely monitor vitals and fluid removal rates (Ultrafiltration) to prevent intradialytic complications like hypotension or cramping.
  • Medication Administration: Administer dialysis-related medications such as Erythropoietin (EPO), Vitamin D analogs, and anticoagulants (Heparin).
  • Lab Correlation: Review monthly labs (KTV, phosphorus, calcium, potassium) and collaborate with the interdisciplinary team to adjust treatment plans.
  • Emergency Response: Recognize and intervene in dialysis-related emergencies, such as air embolisms or blood leaks.

 

Requirements & Qualifications

  • Experience: Minimum 1 year of Dialysis experience is required.
  • Licensure: Active New York (NY) State RN License.
  • Certifications: BLS (AHA) is required.
  • Scheduling: High Flexibility Required. You must be available to work according to the clinic's census needs. No blocked schedules or scheduling requests will be considered.

Unit & Facility Details

  • Care Setting: Post-Acute / Outpatient Chronic Clinic.
  • Clinic Operations: Open 6 days a week (Monday–Saturday).
  • Clinic Hours: 06:00 – 20:30.
  • Shift Length: Up to 13.5 hours depending on the patient schedule.
  • Equipment: 32 Machines / 134 Patients.
  • Charting: DCI-specific computerized system.

 

Clinical Spotlight: The Hemodialysis Circuit

 

As a Dialysis RN, you are the guardian of the extracorporeal circuit. Understanding the mechanics of the dialyzer—the "artificial kidney"—is essential. You must ensure proper blood flow rates (QB) and dialysate flow rates (QD) are maintained to achieve optimal clearance (Kt/V) for the patient while ensuring the machine’s safety alarms (air detectors, pressure monitors) are correctly calibrated.
