🚨 Travel Registered Nurse (RN) – Hospice | Iron Mountain, MI 🚨

We are seeking a compassionate and experienced Home Health RN for a travel assignment covering both home health and hospice patients. This role focuses on providing skilled nursing care in patients’ homes, including admissions, recertifications, and ongoing case management. You’ll work independently in the field while supported by a strong clinical and administrative team.
- Start: 5/25 | 13 weeks
- 12-hour days shift
- Recent Home Health experience (within 2 years)
- Strong OASIS documentation skills
- Experience with wound care
🩺 Patient Population
- Home Health & Hospice patients
- Common conditions:
- CHF
- Diabetes
- Renal disease
- Wound care needs
- IV therapy
- Drains/devices management
🔑 Key Responsibilities
- Perform Start of Care (SOC), Recerts, and Resumptions
- Manage ~6 patient visits/day (points-based)
- Provide wound care and IV therapy
- Complete OASIS documentation accurately
- Maintain strong infection control practices in the home
- Participate in on-call rotation
