Job Title: Occupational Therapist (Home Health)
Location: Kenosha & Racine Counties, WI
Duration: 13 Weeks
Shift Details: 8hr Day Shift (40 hours/week) – Monday to Friday, 8:00 AM – 4:30 PM | No weekends, no call, no holidays
Job Summary
The Home Health Occupational Therapist provides patient-centered care to adult and geriatric patients in their homes. The role focuses on improving functional independence, activities of daily living (ADLs), and overall quality of life through evaluation and therapeutic interventions.
Key Responsibilities
- Perform OT evaluations and develop individualized care plans
- Provide therapy focused on ADLs and adaptive equipment use
- Treat patients in home health settings across assigned counties
- Collaborate with RN and PT interdisciplinary teams
- Participate in daily team huddles (via phone)
- Document care accurately in EMR (EPIC preferred)
Patient Population
- Adult and geriatric patients
- Common conditions: post-stroke, post-joint replacement, COPD, CHF, weakness, deconditioning
Caseload
- Expected productivity: 6 points per day
- Evaluation visit: 1.5 points
- Standard visit: 1 point
Required Qualifications
- Active Wisconsin OT License
- BLS Certification
- Minimum 3 years of OT experience (home health preferred; SNF or acute accepted)
- Bachelor’s degree in Occupational Therapy (minimum)
- Valid driver’s license, reliable vehicle, and active auto insurance
Skills Required
- OT evaluation and treatment planning
- ADL training and adaptive equipment use
- Strong communication and patient education skills
- Ability to work independently in home settings
Support Structure
- RN Supervisor
- PT Lead and PT Manager
- Interdisciplinary team (RN, PT, OT)
Technology & Equipment
- EMR: EPIC (preferred)
Floating Requirements
- Coverage within Kenosha and Racine Counties
- Most travel within 20–30 minutes (occasionally up to 1 hour)
Orientation
- First day at Milwaukee office (National Avenue)
- EPIC training included
- Remainder of orientation with preceptor
Additional Information
- Dress Code: Business casual or scrubs (any color)
- Parking: At patient homes
- No weekend, holiday, or on-call requirements
