Description

Job Summary:

We are seeking a qualified Social Worker to provide inpatient support, including psychosocial assessments, discharge planning, and care coordination. The role involves working closely with the Child Protection Team (CPP) and may require local travel within Madison. The ideal candidate will have strong clinical, communication, and crisis management skills.


Job Details:

  • Schedule: Monday – Friday, 8:00 AM – 4:30 PM
  • Shift: 8-hour day shifts
  • Weekend/On-Call: None
  • Travel: May be required within Madison (with Child Protection Team)

Key Responsibilities:

  • Conduct biopsychosocial assessments and intake screenings
  • Develop and implement patient-centered care and treatment plans
  • Provide crisis intervention and emotional support to patients and families
  • Coordinate discharge planning and post-acute care services
  • Collaborate with physicians, nurses, and multidisciplinary teams
  • Work with child/adult protective services and guardianship cases
  • Assist with advance directives and care planning
  • Connect patients to community resources (Medicaid, housing, transportation, etc.)
  • Educate patients and families regarding care plans and services
  • Maintain accurate and timely documentation in EMR systems

Required Qualifications:

  • Master’s Degree in Social Work (MSW)
  • Minimum 2 years of inpatient social work experience
  • Active Wisconsin License (one of the following):
    • LCSW (Licensed Clinical Social Worker)
    • CISW (Certified Independent Social Worker)
    • CAPSW (Certified Advanced Practice Social Worker)
  • Strong experience in discharge planning and care coordination

Preferred Qualifications:

  • Experience in psychiatric or acute care hospital settings
  • Knowledge of child protective services and crisis intervention
  • Familiarity with community programs and social services
  • Strong understanding of high-risk patient populations
