Licensed Practical Nurse (LPN) – Long Term Care (LTC) | Contract Position

Job Type: Contract
Duration: 13 weeks
Shift: 8-hour varied shifts (days, evenings, nights)
Schedule: Includes weekends and holidays; flexibility required (no guaranteed hours or shift preference)

Job Summary

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide quality care in a Long Term Care (LTC) setting located within a hospital campus. The LPN will support residents with daily care needs, administer treatments, and collaborate with the healthcare team to ensure optimal patient outcomes.

Key Responsibilities

  • Perform patient assessments and monitor changes in condition
  • Administer medications and treatments as prescribed
  • Communicate pertinent patient information to physicians and nurse practitioners
  • Assist with admissions and discharges
  • Provide support with activities of daily living (ADLs)
  • Deliver specialized care including:
    • Gastrostomy tube (G-tube) care
    • Urinary catheter care
    • Nebulized treatments
    • Wound VAC management
    • CPAP therapy
    • Tracheostomy care
  • Maintain accurate and timely documentation
  • Adhere to infection control and safety protocols
  • Perform other duties as assigned

Requirements

  • Active or pending LPN license (must be obtained prior to start)
  • Minimum of 1 year recent Long Term Care (LTC) experience
  • Current BLS (Basic Life Support) certification
  • Ability to work flexible schedules, including weekends and holidays
  • Strong communication and patient care skills

Additional Information

  • Position requires schedule flexibility; shifts and hours are not guaranteed
  • Flu vaccination is required prior to assignment (may be completed during onboarding/employee health clearance)
  • Candidates must be available prior to assignment start for required onboarding and health screening appointments
  • Reference checks will be conducted via email
