Description

Position mainly focuses on support to patients and families to adjust to the idea of going to a long term care- helping them navigate the system and ongoing general support

- Either 44-88 patients

- 40 hours per week

- Monday – Friday

- Primarily 8:00am – 4:30pm

- May need some minor flexibility on shift times – discussed in interview
