Description

Position: RN- Case Manager

Location: Eureka, CA 95501
Job type: 13 weeks of contract with possible extensions
Shift: Night 5x8-Hour (19:00 - 07:00)

"Weekend Requirements: Required to work every other weekend.
**Must have acute care experience!

Years of experience Required: 2 years

HARDSTOP- ACUTE CARE Hospital Case Management EXPERIENCE AND MUST BE CURRENT WITHIN THE LAST YEAR.

REQUIRED:
* Experience with HMOs, IPAs, etc.
*Experience with advance directive, POLST, protective services process
