Description
Job Summary:
We are seeking a Physical Therapist (PT) to provide high-quality rehabilitation services to residents. This role focuses on delivering patient-centered care while working as part of a collaborative in-house rehab team.
Key Responsibilities:
- Provide and direct physical therapy services using evidence-based practices
- Evaluate patients and develop effective treatment plans
- Monitor patient progress and adjust care plans as needed
- Coordinate care with interdisciplinary teams and clinical staff
- Participate in patient/resident screenings as scheduled
- Maintain accurate and timely documentation
- Ensure compliance with state, federal, and regulatory standards
- Maintain billing records as required
- Attend meetings, trainings, and facility initiatives
- Travel to other locations as needed
Requirements:
- Master’s or Doctorate degree in Physical Therapy
- Active Physical Therapist (PT) license in the state of practice
- Strong communication and teamwork skills
Preferred Qualifications:
- Experience in older adult / geriatric rehabilitation
- Knowledge of skilled nursing facility (SNF) or rehab settings
- Familiarity with regulatory and compliance standards
