Now Hiring: Integrated Case Manager (RN)
Location: Detroit, MI (Onsite)
Pay Rate: $51 - $53/hour
Contract Length: 13 Week Contract to Hire Opportunity
We’re looking for an experienced Integrated Case Manager to support patients with complex medical and social needs. This is a hands-on, fast-paced role where you’ll work closely with patients, families, and care teams to improve outcomes and ensure safe, effective care across the entire healthcare journey.
Position Overview:
As an Integrated Case Manager, you’ll play a key role in coordinating care for patients who require advanced support. You’ll assess needs, develop care plans, and help guide patients through the healthcare system—making sure they receive the right care at the right time.
Key Responsibilities:
- Assess patients’ medical, social, and emotional needs to support recovery
- Develop and manage personalized care plans
- Coordinate care across inpatient, outpatient, and community settings
- Support safe and smooth discharge planning
- Educate and advocate for patients and their families
- Collaborate with healthcare teams to manage complex cases
- Connect patients to community resources and support services
- Monitor progress and adjust care plans as needed
Requirements:
- RN (BSN) with an active, unrestricted Michigan license
- 5+ years of case management experience
- At least 2 years of recent acute inpatient case management experience (REQUIRED)
- Strong background in discharge planning
- Ability to work independently with minimal training and adapt quickly
Note: Experience in telephonic case management, utilization review, or behavioral health alone does not meet the inpatient requirement.
Schedule:
- Shift: 8:00 AM – 4:30 PM
- Weekend rotation required
