Facility Name: Columbus Springs East Behavioral Hospital

• Facility Department/Cost Center: Maple, or Sunrise or Laurel

• Shift: 8A-5P

• Weekly Guaranteed Hours: 40 hours

• Weekend Hours: as needed

• Call Requirements: no call

• Required Certifications: LSW,LPC or higher, Oh state license, CPR, HWC

• Years of Experience: 2

• Number of Beds: 24

• Patient Ratios: 1:12

• EHR: pinpoint

• Supervisor: TBD

• Patient Population e.g. “geriatric, adult, peds, etc.”: Adult
