Job Summary:

We are seeking an experienced Home Health RN to support admissions and provide skilled nursing care to geriatric patients in a home setting. The role requires strong clinical judgment, OASIS documentation expertise, and the ability to manage a consistent caseload independently.

Key Responsibilities:

• Perform Start of Care (SOC) / Admissions and initial patient assessments
• Complete accurate OASIS documentation (SOC, Recertification, Resumption of Care, Discharge)
• Manage daily caseload of ~6 patients (30 points/week productivity)
• Provide skilled nursing care including IV therapy, wound care, and medication management
• Monitor patient condition and update care plans accordingly
• Educate patients and caregivers on disease management and treatment plans
• Coordinate care with interdisciplinary teams and community resources
• Maintain timely and compliant documentation
• Follow infection control and safety protocols

Requirements (Must Have):

• Active RN License – Maryland
• 2–3 years of recent Home Health / Skilled Home Care experience
• OASIS documentation experience (required)
• Prior acute care or long-term care experience
• BLS Certification
• Valid Driver’s License + Auto Insurance + Reliable Vehicle
• Ability to manage 6 patients/day in a field setting

Preferred Qualifications:

• Prior travel experience
• Experience in community or teaching hospitals
• Strong background in case management and admissions

Clinical Skills Required:

• OASIS documentation and clinical assessment
• IV therapy, phlebotomy, and central line care
• Wound care and wound VAC management
• Catheter care, feeding tube management
• Medication administration and pain management
• Respiratory care (oxygen, nebulizers)
• Palliative and end-of-life care

Work Details:

• Patient Population: Geriatric
• Setting: Adult Home Health
• Productivity: 30 points/week (~6 visits/day)
• Weekend Requirement: May be required
• On-Call: May be required
