Description
A-Line Staffing is now hiring an RN Home Health in Denver, CO. This position offers competitive pay and the opportunity to provide skilled nursing care to patients in their place of residence while coordinating with an interdisciplinary care team. This is a great opportunity for experienced home health RNs who are comfortable traveling locally and supporting on-call needs as required.
RN Home Health Compensation
- Pay Rate: $58/hr
Schedule
- Shift: 40 hrs/week (flexible schedule; on-call may be required if requested based on branch needs)
Additional Information
- On-site / field-based role (patient care in the home)
- Local travel required (must have reliable transportation/insurance)
- Expected weekly productivity: 30 units
- Float required: Yes
- On-call needs are subject to change based on staffing/branch needs
- Must maintain accurate, timely documentation to support continuity of care and payer requirements
RN Home Health Responsibilities
- Administer skilled nursing care for clients of all ages in their place of residence
- Assess patients in stable and emergent situations to identify physical, psychosocial, and environmental needs
- Develop, implement, document, and evaluate the Plan of Care (POC) to ensure quality and continuity
- Maintain up-to-date clinical records reflecting visits, contacts, problems, actions, and changes in condition
- Provide care utilizing infection control measures that protect staff and patients (OSHA)
- Educate patients/families on care plans, diet, medications, and home safety; promote self-care practices
- Supervise and provide clinical direction to support staff to ensure quality and continuity of services
- Assess/reassess pain, implement pain management techniques, and educate patient/family on pain control
- Arrange referrals to appropriate agencies as needed
- Monitor assigned cases for compliance with third-party payer requirements
- Provide effective communication and serve as a liaison with patients/families, physicians, and care team members
- Perform on-call responsibilities and provide on-call services as assigned
RN Home Health Requirements
- Active RN license in the state (required)
- Graduate of an accredited ADN program or Nursing Diploma (required)
- 1–2 years RN experience in a clinical care setting (required)
- 2+ years home health experience (required)
- Homecare Homebase (HCHB) experience (required)
- OASIS experience (required)
- Proof of current CPR certification (required)
- Current driver’s license (required)
- Reliable transportation and insurance (required)
- Knowledge of medications and correct administration; venipuncture/IV care competency
- Ability to work independently, stay organized, and handle a flexible schedule and local travel
Preferred Qualifications
- CHPN certification (preferred)
- BSN (preferred)
