A-Line Staffing is now hiring an RN Home Health in Cottonwood, AZ. This position offers competitive pay and the opportunity to provide skilled nursing care to patients in their place of residence while coordinating with an interdisciplinary care team. This is a great opportunity for experienced home health RNs who are proficient in Homecare Homebase (HCHB) and OASIS and can participate in an every-other-week on-call rotation.

RN Home Health Compensation

• Pay Rate: $50.87/hr

Schedule

• Shift: Full-time, 40 hrs/week
• On-call rotation: every other week (on-call needs may change based on branch needs)
• Expected weekly productivity: 30 units
• Local travel required

Additional Information

• On-site / field-based role (patient care in the home)
• Must be proficient in HCHB and experienced with OASIS
• Float required: Yes
• Must have valid driver’s license, reliable transportation, and insurance
• Accurate, timely documentation is required to support continuity of care and payer requirements

RN Home Health Responsibilities

• Administer skilled nursing care for clients of all ages in their place of residence
• Coordinate care with the interdisciplinary team, patient/family, and referring agency
• Assess patients in stable and emergent situations to identify physical, psychosocial, and environmental needs
• Develop, implement, document, and evaluate the Plan of Care (POC) to ensure quality and continuity
• Maintain up-to-date patient records reflecting visits, contacts, problems, actions, and changes in condition
• Provide care using infection control measures that protect staff and patients (OSHA)
• Educate patients/families on care plans, diet, medications, home safety, and self-care practices
• Supervise and provide clinical direction to support staff (HHA/STNA/LVN) to ensure quality service
• Assess/reassess pain, apply appropriate pain management techniques, and educate patient/family on pain control
• Arrange referrals to appropriate agencies as needed
• Monitor assigned cases to ensure compliance with third-party payer requirements
• Provide effective communication and serve as a liaison with patients/families, physicians, and care team members
• Provide on-call services as assigned and attend team meetings as needed

RN Home Health Requirements

• Current AZ RN license (required)
• Graduate of an accredited ADN program or Nursing Diploma (required)
• 1–2 years RN experience in a clinical care setting (required)
• 2+ years home health experience (required)
• Homecare Homebase (HCHB) experience (required)
• OASIS experience (required)
• Proof of current CPR certification (required)
• Current driver’s license (required)
• Reliable transportation and insurance (required)
• Knowledge of medications and correct administration; competency with venipuncture/IV care
• Ability to work independently, stay organized, and function under stress

Preferred Qualifications

• BSN (preferred)
• CHPN certification (preferred)
