Job Title

• Licensed Practical Nurse (LPN)

Location

• Brookings, Oregon

Schedule / Job Type

• Full-time, night shifts (Sunday, Monday, Tuesday)

Key Responsibilities

• Provide direct nursing care and support to elderly and acutely ill residents under the supervision of an RN.
• Collaborate with the healthcare team using SBAR communication to assess and recommend resident care plans.
• Administer medications, treatments, and monitor resident responses, ensuring compliance with physician orders and safety protocols.
• Assist with resident evaluations, care planning, and documentation in accordance with legal and organizational standards.
• Assist with emergency procedures, including CPR, and respond promptly to resident needs.
• Coordinate care activities among staff, residents, and families, maintaining clear and timely communication.

Required Qualifications

• Graduate of an accredited LPN program.
• Valid LPN license in Oregon.
• Current CPR certification.
• Ability to react decisively in emergency situations.
• Effective communication skills with residents, families, and staff.

Preferred Experience / Skills

• Six months of experience in a long-term care setting preferred.
• Familiarity with clinical documentation and medication management protocols.
