Description

The Home Health Registered Nurse provides skilled nursing care to patients in their homes, with primary responsibility for OASIS start‑of‑care visits, recertifications, discharges, and ongoing skilled nursing services. This role focuses on accurate assessments, quality documentation, wound care, medication management, and effective communication to support positive patient outcomes while meeting home health standards and productivity expectations.

 

Schedule & Work Details

  • Monday through Friday, 8:00 AM to 4:30 PM
  • Every 3rd weekend - 1 night of on call per week
  • Coverage areas include Luzerne County
  • Mileage reimbursed between patient visits
  • Dress code allows any solid color scrubs

Key Responsibilities

  • Complete RN OASIS Start of Care assessments accurately and in a timely manner
  • Perform RN discharge visits and recertification assessments
  • Provide skilled nursing services including medication set‑ups and wound care
  • Assess patient condition, monitor progress, and identify changes requiring intervention
  • Educate patients and caregivers on disease processes, medications, and care plans
  • Coordinate care with physicians, therapists, and interdisciplinary team members
  • Document all patient care and assessments accurately within Homecare Homebase (HCHB)
  • Ensure compliance with home health regulations, agency policies, and quality standards
  • Promote patient safety and a positive care experience in the home setting

Required Qualifications

  • Current Registered Nurse license
  • Home health nursing experience required
  • Experience with Homecare Homebase (HCHB)
  • Strong clinical assessment and documentation skills
