<h2>Position Overview</h2><p> </p><p>We are seeking an experienced Home Health Registered Nurse to provide skilled nursing care to patients in their homes throughout the Cottonwood and Sedona, Arizona areas. This role focuses on comprehensive assessments, OASIS documentation, advanced clinical skills, and patient education to support safe, high‑quality outcomes in the home setting.</p><p> </p><h2>Schedule &amp; Work Details</h2><ul><li><strong>Schedule:</strong> Day shift, 8:00 AM to 4:30 PM</li><li><strong>Weekly Hour Guarantee:</strong> 36 hours</li><li><strong>Productivity Expectation:</strong> 30 points per week</li><li><strong>Coverage Area:</strong> Cottonwood and Sedona, AZ</li><li><strong>On‑Call/Weekend Requirement:</strong> Yes, on a rotating basis</li><li><strong>Mileage Reimbursement:</strong> Yes, reimbursed between patient visits</li><li><strong>Pay Rate:</strong> $51 to $53 per hour</li></ul><h2>Key Responsibilities</h2><ul><li>Perform skilled RN home visits including comprehensive patient assessments and ongoing evaluations</li><li>Complete accurate and timely OASIS documentation in Homecare HomeBase (HCHB)</li><li>Provide wound care, medication management, and disease process education</li><li>Manage advanced clinical tasks including PICC lines, central lines, catheter and drain care, infusion therapy, and lab draws</li><li>Educate patients and caregivers to promote safety, independence, and reduced hospitalizations</li><li>Communicate changes in patient condition to physicians and interdisciplinary team members</li><li>Maintain compliance with agency policies, infection control standards, and state and federal regulations</li></ul><h2>Required Qualifications</h2><ul><li>Active Registered Nurse license</li><li>Prior home health nursing experience</li><li>Proficiency with Homecare HomeBase (HCHB)</li><li>Strong clinical assessment, documentation, and time management skills</li><li>Ability to work independently in a field‑based environment</li></ul><h2>Preferred Qualifications</h2><ul><li>Advanced home health clinical skill set</li><li>Strong patient education and care coordination abilities</li></ul>
