Description

Shift:

  • Nights: 7:00 PM – 7:00 AM

Job Summary:
This RN role focuses on delivering patient-centered care in a healing and wellness-focused environment. Nursing practice is guided by evidence-based care, hospital policies, regulatory standards, and professional nursing guidelines. The RN is responsible for coordinating and providing direct patient care while collaborating with the interdisciplinary healthcare team.

Key Responsibilities:

Patient Assessment & Care Planning

  • Perform and document comprehensive patient assessments
  • Identify patient risk factors and prioritize patient needs
  • Collect objective and subjective patient data
  • Consider patient culture, age, and developmental level in care planning
  • Develop individualized plans of care in collaboration with patients, families, and the healthcare team

Direct Patient Care

  • Implement nursing interventions, therapies, and procedures within scope of practice
  • Monitor and respond to changes in patient condition
  • Provide patient and family education regarding treatments and care plans
  • Alleviate patient stress and discomfort through compassionate care
  • Conduct ongoing assessments and document patient responses to treatment

Coordination & Leadership

  • Delegate tasks appropriately and oversee patient care activities completed by team members
  • Monitor quality and completeness of delegated care
  • Assist with admissions, transfers, and discharges
  • Coordinate discharge planning with Case Management and interdisciplinary teams
  • Provide input into staffing and patient assignments based on acuity and unit needs

Quality & Professional Practice

  • Participate in quality improvement initiatives and core measures
  • Support patient safety goals including fall prevention, pressure ulcer prevention, and documentation compliance
  • Promote a positive team environment and professional nursing excellence
  • Utilize evidence-based practice and holistic patient care approaches
