<h2>Position Overview</h2><p>The Home Health Registered Nurse provides skilled nursing care to patients in their homes in accordance with physician orders, state and federal regulations, and agency policies. This role is responsible for coordinating and managing the total plan of care in a point-of-care setting, ensuring continuity of care through collaboration with physicians, interdisciplinary team members, patients, families, and community resources. The RN delivers high-quality, patient-centered care while maintaining compliance with documentation, quality, and regulatory requirements.</p><h2> </h2><h2>Schedule &amp; Work Details</h2><ul><li>Schedule: Monday through Friday</li><li>Hours: 8:30 am to 4:30 pm</li><li>Weekly Hour Guarantee: 36 hours</li><li>Coverage Areas: King County and Snohomish Counties</li><li>On Call/Weekend Requirement: Yes, includes weekend rotation and potential availability on observed holidays</li><li>Mileage Reimbursement: Yes, reimbursed between visits from first patient to last patient</li><li><strong>Pay Rate: $51 to $53 per hour</strong></li></ul><h2>Key Responsibilities</h2><ul><li>Clinical Competence</li><li>Initiate, develop, implement, and revise individualized plans of care in collaboration with physicians and the interdisciplinary care team</li><li>Provide direct and indirect supervision of home health aides and LPNs, ensuring appropriate delegation and compliance with state and federal regulations</li><li>Complete required supervisory visits and provide clinical instruction as needed</li><li>Documentation &amp; Care Delivery</li><li>Deliver high-quality clinical services within scope of practice and infection control standards</li><li>Accurately and timely complete comprehensive assessments, including OASIS and medication reconciliation</li><li>Ensure all visit documentation is thorough, compliant with payer requirements, and synced timely per agency policy</li><li>Utilize PointCare documentation systems effectively</li><li>Quality &amp; Compliance</li><li>Conduct initial and comprehensive evaluations to confirm home health eligibility and medical necessity</li><li>Identify the primary focus of care and develop physician-approved plans of care within state-specific guidelines</li><li>Communicate patient care needs, visit assignments, scheduling updates, OASIS data, and orders accurately and timely with agency staff</li><li>Collaborate closely with physicians, patients, and families to ensure quality care and service excellence</li><li>Participate in utilization management activities and efforts to reduce unnecessary hospitalizations</li><li>Professionalism &amp; Teamwork</li><li>Demonstrate professionalism, punctuality, and a positive attitude in all patient and team interactions</li><li>Participate actively in meetings, case conferences, trainings, and survey readiness activities</li><li>Assist with orientation and preceptorship of new staff and students</li><li>Support performance improvement initiatives focused on patient safety, outcomes, and regulatory compliance</li><li>Maintain schedule flexibility to meet patient care demands</li></ul><h2>Required Qualifications</h2><ul><li>Active, unencumbered Registered Nurse license in Washington State or valid Multistate license</li><li>Current Basic Life Support (BLS) certification</li><li>Ability to work independently in a field-based home health setting</li><li>Valid driver’s license and reliable transportation</li></ul>
