Position Overview

The RN Case Manager is responsible for coordinating and managing patient care in the home health setting. This role focuses on comprehensive case management, ensuring high quality, patient centered care while meeting productivity and documentation standards.

Schedule & Work Details

• Coverage Areas: Nash, Edgecombe, and Wilson Counties
• Schedule: Monday through Friday, 8:00 AM to 4:30 PM
• Weekly Hour Guarantee: 36 hours
• On Call/Weekend Requirement: No
• Mileage Reimbursement: Yes, per contract company policy
• Dress Code: Scrubs, no specific color required
• Pay Rate: $51 to $53 per hour

Key Responsibilities

• Manage a caseload of home health patients as an RN Case Manager
• Complete comprehensive patient assessments and ongoing reassessments
• Develop, implement, and update individualized care plans
• Coordinate care with physicians, therapists, and interdisciplinary team members
• Ensure compliance with home health regulations and agency policies
• Monitor patient progress and adjust care plans as needed
• Provide patient and family education related to diagnoses, medications, and care plans
• Maintain accurate, timely clinical documentation
• Meet established productivity expectations while maintaining quality of care

Required Qualifications

• Active Registered Nurse license
• Previous experience in home health or case management
• Strong clinical assessment and care coordination skills
• Ability to work independently in the field
• Valid driver’s license and reliable transportation

Preferred Qualifications

• Experience with Homecare Homebase (HCHB)
• Strong organizational and time management skills
• Excellent communication and patient engagement abilities
