Description

Position Overview

We are seeking an experienced Registered Nurse to provide skilled home health nursing services to patients across Robeson, Hoke, and Cumberland counties. This role focuses on delivering high‑quality, patient‑centered care in the home setting while ensuring compliance with physician orders, agency policies, and state and federal regulations.

 

Schedule & Work Details

  • Schedule: Monday through Friday, 8:00 AM to 4:30 PM
  • Weekly Hour Guarantee: 40 hours
  • Productivity Expectation: 30 points per week
  • Coverage Area: Robeson, Hoke, and Cumberland counties
  • On‑Call/Weekend Requirement: Yes
  • Mileage Reimbursement: No
  • Pay Rate: $51 to $53 per hour

Key Responsibilities

  • Provide skilled nursing care to patients in their homes in accordance with physician orders and individualized plans of care
  • Perform comprehensive patient assessments and ongoing evaluations
  • Deliver skilled nursing interventions including medication administration, patient education, and clinical monitoring
  • Coordinate care with physicians and interdisciplinary team members and promptly report changes in patient condition
  • Accurately document all visits, assessments, and care provided in the electronic medical record
  • Maintain compliance with agency policies, infection control standards, and state, federal, and CMS guidelines

Required Qualifications

  • Active and unrestricted Registered Nurse license
  • Ability to work independently in a field‑based environment
  • Strong clinical judgment, assessment, and documentation skills
  • Preferred Qualifications

Previous home health experience

  • Experience using Homecare HomeBase (HCHB)
  • Strong time management skills and ability to manage a mobile caseload
