Location: Pittsburgh, PA 15206
Pay Rate: $35.00/hour
Shift: 2:00 PM – 11:00 PM
Assignment Duration: 6 Months+
Schedule: Full-time
Employment Type: Temporary to Permanent (opportunity to extend or go permanent based on business needs)
Work Environment: Skilled Nursing / Long-Term Care Veterans Center

Job Summary:
The Licensed Practical Nurse (LPN) performs all aspects of general nursing care and clinical duties within the scope of licensure and training. The LPN will provide direct patient care, administer medications and treatments, support resident wellbeing, maintain accurate documentation, and collaborate with the healthcare team to ensure quality patient care in a skilled nursing and long-term care veterans facility.
Key Responsibilities:
- Administer medications and treatments accurately and timely as ordered by a physician.
- Order and receive medications per procedure.
- Count narcotics and verify records for accuracy and completeness.
- Check medications from pharmacy for accuracy and report any discrepancies.
- Participate in feeding residents and communicate concerns to the RN in charge.
- Provide specialized care to residents on specialty units, including Alzheimer’s care units.
- Conduct rounds and verify resident census to ensure safety and wellbeing.
- Observe resident conditions and report changes to appropriate staff.
- Document daily care activities in approved electronic medical records.
- Assist with resident admission documentation and consultation records.
- Record appointments, chart medications, and treatments according to procedures.
- Forward medical records upon resident transfer when required.
- Collaborate with RNs in developing and implementing resident care plans.
- Participate on nursing committees and quality improvement initiatives.
- Ensure residents are clean, groomed, and receiving appropriate dining room care.
- Receive and provide detailed shift reports.
- Observe residents for injuries and notify RN Supervisor of concerns.
- Communicate activity participation restrictions and clearances for residents.
- Follow all infection control and universal precaution guidelines.
- Assist RN with daily resident health assessments and maintain confidentiality.
- Assist with orientation and monitoring of LPNs and CNAs.
- Monitor nursing care supply levels and communicate needs to leadership.
- Attend required meetings, conferences, seminars, and mandatory training programs.
- Support QAPI initiatives and participate in assigned PIP teams.
- Perform CPR and respond to emergencies as appropriate.
- Sterilize supplies and instruments using approved methods.
- Administer EKGs, visual acuity exams, and related procedures.
- Assist residents with activities of daily living and hygiene needs.
- Prepare medical incident reports related to injuries or sudden illness.
- Perform additional related duties as assigned.
Physical Requirements:
- Ability to lift, reposition, transfer, and ambulate residents.
- Ability to push and pull wheelchairs and medication carts.
- Manual dexterity required for direct patient care.
- Ability to hear adequately for communication and stethoscope use.
- Corrected vision sufficient to visually assess residents and maintain safety.
- Ability to negotiate stairways within resident care units.
Required Qualifications:
- Current Pennsylvania Licensed Practical Nurse (LPN) license or eligibility to sit for the Practical Nurse Licensure Examination.
- Current CPR certification or ability to obtain and maintain certification.
- Proficient computer skills, including Microsoft Office products.
- Ability to accurately enter health data into electronic medical records systems.
- Strong verbal and written communication skills.
- Ability to work overtime as required by management.
- Ability to maintain professionalism in a skilled nursing and long-term care environment.
