Location: Rockford, IL 61114
Pay Rate: $53.00 - $58.00/hour
Assignment Start Date: May 2026
Assignment End Date: August 2026
Employment Type: Temporary to Permanent (opportunity to extend or go permanent based on business needs)
Schedule: Days, 40 hours per week

Job Summary:
The Registered Nurse Diabetes Educator provides compassionate, culturally sensitive, and holistic nursing care to patients in a clinical setting while supporting diabetes education and disease management initiatives. In collaboration with a multidisciplinary diabetes management team, this role provides education and support to patients and families living with diabetes mellitus while promoting positive patient outcomes and continuity of care.
Key Responsibilities:
- Utilize the nursing process and collaborate with interdisciplinary team members in patient care.
- Administer prescribed medications and treatments in accordance with approved nursing techniques, policies, procedures, standards of care, and state practice regulations.
- Document patient care according to established standards of care.
- Develop, initiate, and execute individualized patient care plans as prescribed by physicians and clinic policies.
- Provide patient and family education related to diabetes management and treatment plans.
- Participate in department and unit performance improvement initiatives and shared governance activities.
- Utilize clinical expertise to support patient longitudinal care planning across departments and units.
- Apply critical thinking skills related to patient safety and patient experience while escalating concerns appropriately.
- Participate in professional nursing development and continuing education activities.
- Serve as a resource to staff regarding patient education programs, educational materials, and patient education concerns.
- Advocate for patients regarding clinical, psychosocial, and financial needs.
- Float to various units based on operational needs and clinical competency.
- Work variable shifts, weekends, holidays, and/or on-call hours if required.
- Perform additional duties as assigned.
Required Qualifications:
- Graduate of an accredited nursing program.
- Education verification is required.
- Active Registered Nurse (RN) license in both Illinois and Wisconsin.
- Approved CPR certification required within 90 days of hire.
- Current CDCES (Certified Diabetes Care and Education Specialist) certification required.
- Strong clinical assessment and patient education skills.
- Ability to collaborate effectively within a multidisciplinary healthcare team.
- Excellent communication, documentation, and critical thinking skills.
- Ability to adapt to changing patient care needs and float between units as needed.
Preferred Qualifications:
- Bachelor of Science in Nursing (BSN) preferred.
