Shift: 8hr Day Shift (40) | Description: RN HH Case ManagerAbout Facility:Home office in Oakbrook, IL-Field work South West ChicagolandHome Health position; see coverage area belowUnit Information:Minimum Years of Experience required? 2 years of experience, at least 1 year needs to be Home Health Will you accept a first-time traveler? Will considerPatient Types/Common Diagnoses:Chronic Disease management (CHF, COPD, DM), Foley Catheters, Wounds, PICC/Infusion, LVADCase Load:6 visits/day unless taking 1 Oasis visit, then will adjustRequired License, National Certification, Certs:AHA BLS, driver’s license, insurance, vehicleSkills required:Wound care/management, foleys, PICC/Infusion, comfortable working with interdisciplinary team, central/PICC line dressing changes, de-accessing portsEPIC experience required (will take any EPIC experience) Support within the Department:Manager/Supervisor available via phoneHome Health Aide available if needed (not in the same visit with RN)Will schedule own active patients; scheduler will schedule new patientsTechnology/Equipment:EMR: EPIC Remote Client – any EPIC experience is required IV Pump: Saphire Unit specific equipment: will receive laptop, cell phone, and trunk stock Floating: If yes, where?Yes, see below for coverage territory. In rare instances, may overflow to zip codes 60617, 60619 60620, 60643, 60628 if primary area needs changeOrientation (facility and unit): NEOUp to 2 weeks of unit orientation. EPIC onboarding followed by preceptor week 2 and can provide additional as warranted for competency Scheduling:Weekend rotation: Every 3rd weekendOn call? None Holiday Expectations: None, can work weekend Sunday before or Saturday after if not taking unpaid holiday offApproval of RTO: to go through manager, Max 7 daysShift times: 0800 until last patient is completeSchedule cycle: Monday – Friday, unless it’s their weekend to work. Will adjust schedule accordingly Other notes:Scrub Color: Any solid color, professional. No graphics, logos, designsParking: Dependent on patient housingWill have 8am morning huddles remotely via Teams.
