Description

Position Overview

The Home Health Registered Nurse provides skilled, patient-centered nursing care to individuals in their homes in accordance with physician orders, agency policies, and state and federal regulations. The RN works independently in the field while coordinating care with the interdisciplinary team to promote positive patient outcomes, safety, and continuity of care across the assigned service areas.

Schedule & Work Details

  • Schedule: Monday through Friday
  • Hours: 8:00 am to 4:30 pm
  • Weekly Hour Guarantee: 32 hours
  • Coverage Areas: Muskingum, Licking, and Fairfield Counties
  • On Call/Weekend Requirement: Every 6th weekend rotation
  • Mileage Reimbursement: Yes, reimbursed between patient visits from first to last visit
  • Pay Rate: $51 to $53 per hour

Key Responsibilities

  • Provide skilled nursing care in the home health setting in accordance with physician orders and established plans of care
  • Perform comprehensive patient assessments, including start of care, follow-up, recertification, and discharge visits
  • Develop, implement, and update individualized plans of care based on patient condition and changing needs
  • Administer medications, monitor patient responses to treatment, and provide skilled nursing interventions as appropriate
  • Observe, assess, and document changes in patient condition and communicate findings promptly to the care team and physicians
  • Coordinate care with physicians, therapists, case managers, and other interdisciplinary team members
  • Educate patients and caregivers on diagnoses, medications, treatment plans, and home safety
  • Complete accurate, timely clinical documentation in compliance with agency policies and payer requirements
  • Maintain productivity, quality, and compliance standards
  • Participate in on-call weekend rotation as scheduled

Required Qualifications

  • Active, unencumbered Registered Nurse license
  • Current Basic Life Support (BLS) certification
  • Ability to work independently in a field-based environment
  • Strong clinical assessment, time management, and communication skills
  • Valid driver’s license and reliable transportation
