Long Term Care Registered Nurse (LTC RN) – Contract

We are seeking a compassionate and dependable Long Term Care Registered Nurse (LTC RN) to join a patient-focused healthcare team in a long-term care setting located on a hospital campus. The ideal candidate will have strong clinical skills, flexibility with scheduling, and experience caring for residents requiring a wide range of nursing interventions and daily support.

Position Overview

The LTC RN is responsible for providing skilled nursing care to long-term care residents, including assessments, medication administration, treatments, care coordination, and support with activities of daily living. This role requires excellent clinical judgment, strong communication skills, and the ability to adapt to varied patient needs in a collaborative healthcare environment.

Key Responsibilities

• Perform comprehensive patient assessments and ongoing monitoring
• Administer medications and treatments according to physician orders and facility protocols
• Communicate pertinent patient condition changes to physicians and nurse practitioners
• Assist with admissions, discharges, and care planning
• Provide support with activities of daily living (ADLs)
• Deliver specialized nursing care including:
  • Gastrostomy tube care
  • Urinary catheter care
  • Nebulizer treatments
  • Wound vac management
  • CPAP care
  • Tracheostomy care
• Maintain accurate and timely documentation within the electronic medical record system
• Collaborate with interdisciplinary healthcare teams to ensure quality resident care
• Follow infection control, safety, and facility policies and procedures
• Perform additional nursing duties as assigned

Qualifications

• Minimum of 1 year of Long Term Care nursing experience preferred
• Strong assessment and clinical decision-making skills
• Ability to manage multiple patient needs in a fast-paced environment
• Flexibility to work varied shifts, weekends, and holidays
• Experience with EPIC EMR preferred
• Ability to work effectively as part of an interdisciplinary healthcare team

Licensure & Certifications

• Active Hawaii RN license required by assignment start date
• Candidates without an active Hawaii license must apply upon offer acceptance
• Current BLS/BCLS certification required

Work Environment

• Long Term Care setting located within a hospital campus
• Varied 8-hour shifts
• Weekend and holiday coverage required
• Flexible scheduling required; shift preference and hours are not guaranteed
