Case Manager - LVN – Days
Start Dates: 6/1
Orientation Dates: 6/1(Paid Orientation—3 days)
 

• 13-week contract
• 40 hours
• 5 x 8 ’s
• Shift: Days
• 9am-5:30pm with alternating weekends
• Patient ratios (if known):
• Within Guidelines of CA State Law
• Float Requirements: None
• Is RTO Accepted? What is the Max:
• Up to 4 days max RTO approved
• On Call Requirement? NO
• Certs: BLS, LA County Fire Card
• License: Active CA LVN License
• If CA License is expired, it will need to be renewed
• Experience:
• Required Work Experience: 1 year of Case Manager experience and 2 years of LVN experience
• Experience required: Discharge Planning, Case Coordination, will be working closely with the Case Manager and Nurses.
• More of a Coordinator. Nothing bedside, someone who has experience with placements, dispositions, home comfort.
• Excellent and clear communication skillset required
• Hospital needs clear and meaningful communicators to improve their outcomes
• Previous employees who were terminated will not be accepted for submission or placement
• Travel Experience not required but preferred
• Orientation: The LVNs will complete a structured 5-day training program at the facility which includes 2 days of required online education followed by 3 days of orientation with a preceptor to support hands-on learning and integration into the team. We are available to begin orientation starting June 1, and we look forward to partnering closely to ensure a smooth and successful onboarding experience for the LVNs.
