Description

Shift: Evening Shift (3:00 PM – 11:00 PM)
Hours: 8 Hours/Day

Job Responsibilities

Monitor residents for changes in physical and mental condition and report concerns promptly.
Administer medications, IV therapy, oxygen, tube feedings, wound care, and treatments per physician orders.
Maintain accurate electronic MAR/TAR documentation and resident records.
Supervise LNAs/CNAs and oversee resident care assignments.
Assist physicians during rounds and communicate resident care updates.
Handle admissions, discharges, transfers, infection control oversight, and unit management duties.
Communicate with residents’ families regarding care updates and concerns.
Ensure compliance with New Hampshire nursing regulations and facility policies.

Required Qualifications Minimum 3 years of current LTC experience within the last 6 months. Active New Hampshire LPN license (must be in hand at submission). BLS certification through American Heart Association. IV certification or willingness to obtain during onboarding. Must be willing to float to units where experienced. COVID vaccination required (no exemptions accepted).
