Description

Job Title: Registered Nurse – Case Manager

Location: Eureka, CA

Experience Required: 2+ Years

Job Description:
We are seeking an experienced Registered Nurse – Case Manager for an acute care hospital setting in Eureka, CA. The ideal candidate will have at least 2 years of recent acute hospital case management experience with strong skills in care coordination and discharge planning in a fast-paced environment.

Requirements:

Active Registered Nurse (RN) license
Minimum 2 years of acute hospital Case Management experience
Experience performing Care Coordination and Discharge Planning
Experience working with HMOs, IPAs, and related insurance systems
Ability to independently arrange DME, SNF placement, and ambulance transportation
Strong communication and organizational skills
Ability to organize and participate in family meetings
Participate in development and coordination of patient treatment plans
Experience with computerized documentation systems required

Additional Information:

Start Date: ASAP
First-time travelers accepted with Clinical approval
Weekend Requirement: 2 weekends every 6 weeks
On-call not required
Floating between similar units may be required based on facility needs
