Description

Responsibilities:

Evaluate and treat patients with physical function disorders and wound care needs.
Develop and adjust individualized treatment plans using evidence-based practices.
Perform rehabilitation procedures including therapeutic exercises, ambulation, manual therapy, modalities, and wound care.
Monitor patient progress and document treatments accurately in EMR.
Educate and counsel patients, families, and caregivers on treatment plans and recovery.
Communicate effectively with healthcare teams regarding patient care and progress.
Complete discharge summaries and maintain compliance with regulatory standards.
Record treatment minutes and maintain accurate patient records.
Participate in care conferences, meetings, trainings, and in-services.
Coordinate necessary DME/supplies and supervise PT assistants, aides, and students.
Follow APTA guidelines, state practice acts, and professional ethics standards.

Requirements

1 year of PT experience
New Grads OKAY- as long as a Level II experience and not just a Level I observation experience
LTC or Rehab experience
MA State License
