Licensed Master Social Worker (LMSW) – Home Health

Position Overview

We are seeking an experienced and compassionate Licensed Master Social Worker (LMSW) for a 13-week Home Health travel assignment in East Portland, Oregon. The LMSW will provide psychosocial assessments, care coordination, discharge planning, and patient advocacy for adult and geriatric home health patients in a collaborative interdisciplinary environment.

Shift: Day Shift, 5x8s (8:00 AM – 4:30 PM)
Weekly Schedule: 40 Hours
Patient Population: Adults & Geriatrics
Setting: Home Health
Charting System: Epic Remote Client / Rover / Oasis
Travel Required: Daily local travel with own transportation

Key Responsibilities

Conduct comprehensive biopsychosocial assessments and initial patient evaluations
Develop and revise individualized care plans to meet patient and caregiver needs
Coordinate care with interdisciplinary healthcare teams and community resources
Assist with discharge planning and transitions of care
Educate patients and caregivers regarding available support services and healthcare programs

Requirements

Active LMSW/LCSW licensure
Oregon and Washington licensure required (WA license may be pending at submission)
Minimum 1 year of skilled Home Health experience required
Oasis documentation experience required
BLS certification required
Experience with Epic preferred
